Unit phase alarms

Unit phase alarms can be used to trigger Batch Executive alarms and send a recipe to HOLD. This is accomplished by adding an expression to the Failure_Monitor logic that sets the FAIL_INDEX parameter based on the state of the unit phase alarm.
